1. Download the “My 2022” smartphone application in Google play or Apple APP Store, you can also visit website https://hms.beijing2022.cn. User name is the registration number of your Olympic Identity & Accreditation card. Initial password is the issued date (YYYYMMDD) of your passport.
2. Make sure you are fully vaccinated at least 14 days before travelling to China and upload your vaccination certificate to the HMS in the “My 2022”.
3. Check your health daily for the 14 days before departure for China and track it in the HMS.
4. Take two COVID-19 (PCR) tests in different labs approved by Chinese Embassy/Consulate (see http://ng.chineseembassy.org/eng/zt/BWO/202201/t20220130_10637044.htm) on two separate days (with a minimum 24-hour interval) within 96 hours of the departure of your flight to China (final leg of the flights, if you are travelling indirectly). One of the two tests must be within 72 hours of departure.
5. If your two COVID-19 (PCR) tests taken within 96 hours of departure are negative, upload photo files of the necessary documents listed below to “My 2022” or https://hrhk.cs.mfa.gov.cn/H5/:
– Valid passport or other travel document
– Games accreditation (PVC or OIAC/PIAC) or visa with invitation letter from Beijing 2022
– Test result certificates of 2 tests taken within 96 hours of your departure flight to China from 2 different labs approved by Chinese Embassy/Consulate
6. The Chinese Embassy/Consulate in Nigeria will review the documents and electronically issue the Green Health QR Code that you will need to enter China(see The Playbook - Athletes and Team Officials.pdf).
7. The Chinese Customs requires you to complete the online Customs Health Declaration form at https://health.customsapp.com. Once this is complete you will receive your Customs Health Declaration QR Code.
8. You will need both Green Health QR Code and Customs Health Declaration QR Code for travel to China.
9. If you experience any symptoms of COVID-19 in the 14 days before departure for China or have previously been infected with COVID-19, contact Beijing Winter Olympics Organizing Committee （phs_1@beijing2022.cn）and inform your CLO. They will tell you how to do.
10. Keep your physical interactions to a strict minimum during the 14 days before you travel to China.
