Requirements for the Health Declaration Form Application for Foreign Passengers to China

The following are the latest requirements for the Health Declaration Form application for foreign passengers to China, which will be effective from 18th Nov, 2022 (included).

1. Direct flight from Nigeria to China

Take ONE nucleic acid test within 48 hours before departure (based on sample collection time), if the result is negative, then apply for the Health Declaration Form at https://hrhk.cs.mfa.gov.cn/H5/ .

Passengers departing from Abuja should apply for the Health Declaration Form from the Chinese Embassy in Nigeria.

While Passengers departing from Lagos should apply for the Health Declaration Form from the Consulate-General of China in Lagos.

2. About transit passengers

There is no limitation on the routes or times of flight transfer.

Passengers can take ONE nucleic acid test either in Nigeria or in the place of their transit. If you have taken the test and gotten the Health Declaration Form in Nigeria, you do not need to retake or reapply in the place of your transit.

If a direct flight to China is not originating from Nigeria, the ONE nucleic acid test should be taken within 48 hours before the direct flight to China departs from the transit country. You should apply for the Health Declaration Form from the Chinese Embassy or Consulate in the place of your transit. In addition, we strongly recommend you visit the websites of the Chinese Embassy or Consulate for more information.

3. The Chinese Embassy in Nigeria and the Consulate-General in Lagos do not designate nucleic acid testing institutions. Any legitimate nucleic acid report from the nucleic acid institutions approved by the Nigerian government is acceptable.

4. There are no additional requirements for passengers with a Covid-19 infection record, suspicious symptoms, uncertain test results, or close contacts with confirmed cases. All passengers can apply for the Health Declaration Form with a negative test result within 48 hours before their direct flight to China.

5. Apply at https://hrhk.cs.mfa.gov.cn/H5/ and upload soft copies of the following documents:

(1) One negative nucleic acid test result;

(2) Passport data page;

(3) Chinese VISA page;

(4) Travel itinerary from Nigeria to China;

(5) Photos of the applicant while the specimen is being collected for the nucleic acid test.
